Точная и ранняя диагностика грибковых кератитов остается серьезной проблемой. Клинические проявления инфекционных кератитов очень разнообразны. Практически ни один клинический признак в отдельности не может рассматриваться как патогномоничный для той или иной конкретной инфекции. Но сочетание нескольких признаков значительно повышает вероятность постановки верного диагноза. В одном из исследований было показано, что вероятность верного диагноза грибкового кератита только по фотографиям специалистами по роговице может составлять только около 66% [1]. Однако в другом ретроспективном исследовании отмечено, что у обученных офтальмологов вероятность правильной постановки диагноза может достигать 94% [64]. Достоверно диагноз не может быть поставлен только по клиническим проявлениям, для подтверждения необходимо проведение микробиологических исследований. Однако в условиях малой информативности и доступности инструментально-лабораторных методов исследования правильная оценка клинической картины и возможность по ней заподозрить небактериальную инфекцию может позволить врачу-офтальмологу правильно выбрать лечебную тактику и подбор первичной эмпирической терапии. Учитывая, что благополучный исход лечения грибковых кератитов во многом зависит от своевременно начатого правильного лечения, требуется с одной стороны обучение офтальмологов правильной интерпретации возможных признаков грибковых кератитов, а с другой стороны дальнейшее совершенствование и более широкое внедрение современных методов оперативной малоинвазивной, высокочувствительной и специфичной диагностики этиологических факторов инфекционных кератитов.
